Our board-certified Nurse Practitioners provide services typically performed by outside physicians at your location and we are available 24/7. You are paired with the same Nurse Practitioner, who will visit your community on a regular basis to provide continuity of care and become a trusted health advisor. Our goal is to allow residents to continue living in their current situation and to avoid hospitalizations.

Our services
Our nurses provide the same care you would typically expect in a doctor’s office, but in the comfort of your home. Our services include:
- Telehealth consultations or complete physical exams, including patient histories
- Ordering and interpretation for lab work and diagnostic tests such as X-rays, dopplers, CT scans, MRIs
- Medication review and management, including ordering refills
- Chronic disease management
- Transitional care
- Physician rounding to supplement the NP/PA
- Psychiatric nurse practitioner rounding
- Providing diagnoses and making referrals to specialists, when needed
- Wound care
- DME including hospital beds, oxygen, medical supplies
- Family assistance for end-of-life planning including signing legal paperwork and DNR orders
- Patient counseling for smoking cessation, weight loss, dietary management
- Direct access to collaborating physicians, when needed
