National Church Residences hospice care provides compassionate end-of-life care throughout Ohio for patients with life-limiting illnesses. With 20+ years of experience, our interdisciplinary team delivers pain management, emotional support, spiritual care, and our unique Memorable Moments program in your home or residence. Medicare, Medicaid, and most insurance accepted. Currently available only in Ohio with plans to expand.

Our process is straightforward:

The first step is scheduling an appointment with one of our Hospice Specialists. They assess eligibility and help determine if hospice care aligns with your loved one’s condition and your family’s goals.

At National Church Residences, we understand the difficulty that comes with hospice decisions. We collaborate closely with families to develop personalized care plans that honor your choices about how you want to experience this meaningful time. Our goal is strengthening family relationships while ensuring the best possible outcomes for both patients and loved ones.

Hospice care is often covered by Medicare, Medicaid, and many private insurance plans. Our team can help review your coverage and guide you through options to ensure your loved one receives the care they need.

Our hospice services are currently available only in Ohio. We’re working on expanding into other states and will update our service areas as they become available.

We provide extensive family support including education and training for caregiving tasks, respite care to give caregivers breaks, emotional and spiritual counseling, 24/7 phone support, volunteer assistance, and bereavement services for 13 months after loss. You’re never alone on this journey.
