Service: Community Medical Services

Providing on-site, integrated, person-centered primary care in housing environments allows National Church Residences to demonstrate better care, better health and lower costs.

Community Medical Services (CMS) is an innovative expression of National Church Residences forward-looking integration of housing and health care.

Offered in selected senior, adult day and permanent supportive housing communities, our CMS places Certified Nurse Practitioners (CNPs) on site. The CNPs are licensed to diagnose and treat illnesses and are able to prescribe medications. They offer primary care to help manage chronic diseases, like high blood pressure, congestive heart failure and respiratory problems.

CNPs also help residents and clients to stay well and feel better by offering same-day treatment for common conditions, such as cough and cold symptoms, minor burns and cuts and ear infections. In addition, our CNPs can administer vaccinations, order lab work and perform tests to diagnose and treat many conditions.

CMS offers podiatry services and same-day care for unexpected illnesses, such as:

  • Cold and cough symptoms
  • Minor burns and cuts
  • Minor infections
  • Earwax removal
  • Skin infections
