Molina Healthcare Announces $92,500 Investment in Senior In-Home Assessment Program

Molina Healthcare announces its investment of $92,500 in a National Church Residences’ program serving seniors in their homes, enabling them to remain independent. Home for Life is an in-home assessment and engagement program that addresses social determinants impacting access to healthcare, and connecting seniors with traditional and non-traditional service solutions. National Church Residences will utilize the funds to expand its program to south western Ohio, including the Cincinnati metro area.

The Home for Life program will provide   service coordinators   to care for more than   100 Molina members. These members may be identified as needing additional social supports to better manage their chronic diseases once they are discharged from the hospital or have previously been a patient of the  hospital. According to Senior Vice President for Home and Community Services, Terry Spitznagel, “Our service coordinators are looking forward to serving Molina members in the comfort of their own home and building a trusting relationship with each member. We are committed to meeting the member’s goals, increasing their overall satisfaction with their managed care plan, as well as improving their health and increasing access to services.”

A smaller pilot program National Church Residences conducted with Molina members in 2018, led to demonstrated improved health outcomes, such as 81 percent of individuals received an annual flu shot, an increase by 32 percent from the initial baseline. In addition, 100 percent of members received an annual physician visit, while also getting other preventative health screenings.

“Molina is excited to launch the Home for Life program with the National Church Residences,” said Ami Cole, president of Molina Healthcare of Ohio. “This initiative will allow us to increase members’ access to primary care physicians (PCPs). At least 80 percent of all participating members will receive an annual PCP visit within the first 12 months of their participation in the program, and then increase to 95 percent in the following 12 months.”
