Anthony Evans has joined National Church Residences as its Senior Vice President & Chief Operating Officer for Senior Living. In this role, he leads the Senior Living residential campuses and Home & Community Services.
Evans came to National Church Residences from CareSource Management Group, a subsidiary of CareSource, a not-for-profit health care plan headquartered in Dayton, where he served as Senior Vice President, Integrated Care and Home Health Services.
While at CareSource, he directed the design and implementation of home health and long-term services clinical and operational systems, quality programs, and value-based reimbursement initiatives to improve the quality, effectiveness and sustainability of services.
He is regarded as national expert in population health management and long-term services and support services. One of his most recent career achievements is the successful launch and stabilization of MyCare Ohio Integrated Care Delivery System for Medicare/Medicaid dual eligibles in Ohio.
Prior to his role with CareSource, he owned and operated a Medicare-certified home health and hospice agency in central Ohio, and more recently served as Chief Operations Officer for the largest State-administered Medicaid Waiver Program in Ohio, Carestar.
He also has served as director of nursing at a hospice and home care agency. He began his career as a registered nurse and EMT paramedic. Evans is a graduate of Ohio University and Clark State College.
